Edgewater Fire Rescue is providing Vials of L.I.F.E (Lifesaving information for Emergencies) for the citizens
of Edgewater. This program fully funded by the Edgewater Fire Rescue Association.
The Vial of L.I.F.E. Program is designed to provide Emergency Service Personnel with life saving medical
information.
When medical personnel arrive on a medical emergency they have been trained to look on the refrigerator for
your medical information sheet, which will provide them with medications with dosages, medical history,
allergies, and etc. This sheet will also provide them with important emergency contact information.
The information that this sheet contains is a very important tool for Emergency Medical Services personnel,
especially if you live alone. If something happens to you emergency workers will have the information sheet
that contains your medical history and medication(s) that you take as well as an emergency contact person
you need notified. Even If you don’t live alone, often times loved ones at home are flustered or confused
in an emergency and can’t give accurate vital information.
This form could save your life!
You can get your Vial of L.I.F.E. medical information sheet from fire station 55 at 1605 S. Ridgewood Ave.
or fire station 57 at 2628 Hibiscus Dr., Edgewater.
